Keiser University SHIP/Wellfleet insurance FAQ:
 

1. All student athletes are required to hold active primary health insurance which is accepted by local providers
   1. What does this mean?
      1. All student-athletes that are international, non-insured, or have an out of state HMO or Medicaid insurance plan must opt IN for the SHIP insurance.
   2. How to opt-out or find out if my insurance is accepted?
      1. Go to: https://www.insuranceforstudents.com/student-plan-portal/plans-landing-page?&student_type_id=1&school_id=256&yr=2019
      2. Click ‘Domestic Student Waiver’
      3. Certify and click ‘submit’
      4. Check ‘yes’ for Domestic Student
      5. Fill out all info (make sure you have your insurance card info)
      6. Click Submit waiver
         1. You will be reached out to by a member of the staff weather your insurance was accepted or not. If not, you must get the SHIP insurance.
   3. How to opt-in for the SHIP insurance
      1. Opt-In Instructions (New Students) | Opt-In Instructions (Returners)
         1. There are 2 different sets of instructions for new vs returning athletes so be sure to click on your applicable set of instructions.
   4. Once I have opted-in for SHIP insurance how long does it take for me to receive a copy of my insurance card?
      1. Around 3-5 business days
   5. What do I need insurance wise to be able to participate?
      1. All insurance information for policy INCLUDING a physical copy of your health insurance card.
   6. Who is the SHIP insurance through?
      1. Cigna
   7. What is the coverage period of the SHIP insurance?
      1. August 1, 2024 – July 31, 2025
   8. What are my benefits that I receive with the SHIP insurance?
